How do some ridiculous fights, etc. pass testing?

Sometimes there are fights or other sequences in games that are so ridiculously, clearly problematic I find it hard to understand how they passed testing (i.e. absurdly overpowered bosses). One explanation I saw a developer give once is that their team tested something so long they became amazing at it and didn’t realize that was throwing off their perspective of how others would do. So later they went back and fixed it.

I haven't met with a game developer so I can't really say what goes on during the process, but honestly Even movie producers fail to spot some errors when editing a movie.

For video games maybe they were in a haste to get the game out and failed to properly observe the game before releasing out or maybe they couldn't do anything about it so they had to release it that way.
