Losing companions as a consequence

What are some games where you can dramatically lose a companion as a consequence for your choices? Particularly examples that are a bit ludicrous?

For instance, in Dragon Age 2, I had a companion die because I was one approval point short of the threshold to prevent that from happening.

The ludicrous approval system reminds me of Fire Emblem. Make the wrong dialogue choice or fail to build enough rapport, and characters can die or leave forever. It’s like trying to juggle friendship with battlefield strategy.

Games like Mass Effect's Wrex death for being 2 points short of a high enough approval rating or Divinity: Original Sin 2's Mira execution for being 1 wrong answer away from saving her, are perfect examples of dramatic, ludicrous consequences.

I remembered the whole debacle over Wrex's death over an inadequate approval rating back in the day. Some People thought it was too harsh.

I remember that in the series Mass Effect some choices can cause the death of companions. More specifically in Mass Effect 3 I made the decision to incite the destruction of Thessia, an entire planet and its population and killed off a particular companion in the process.
