How effective are content flagging systems in managing controversial user-generated content?

Content flagging systems aim to curb hate speech, misinformation and explicit material, but thorny challenges persist. How effective have automated filters been in spotting nuanced violations, and where do they falter - contextual sarcasm, evolving slang or adversarial phrasing? What roles do human moderators play in adjudicating edge cases, and how do queue backlogs impact response times? Are there examples where overzealous flagging led to unjust removals, or systems that successfully balanced safety with free expression?

Content flagging systems are often ineffective at managing controversial content.  Subjectivity, bias, and gaming the system undermine their ability to fairly and accurately identify and address problematic posts.  They're often reactive rather than proactive.
